​​​​​​​​​​​​​​​​​​​Before you begin with Baylor Scott & White Health, you'll complete your pre-hire screening at one of our Employee Health hubs.

The hubs offer a convenient, one-stop shop for all your onboarding needs.​ Keep reading for details about what to expect, how to prepare and where to go.

What's included in your pre-hire appointment: 

  • Testing for drugs, nicotine and tuberculosis

  • Providing your Form I-9 documentation

  • Taking your employee ID badge photo

  • Receiving job-specific immunizations

  • Taking fit testing (if applicable)

How to prepare for your appointment:

  • Block your calendar: The appointment may take up to two hours.

  • Arrange for childcare (if applicable): No children are allowed to accompany you.

  • Dress appropriately: Your badge photo will be taken during your visit so wear business attire or any colored scrubs, with no visible piercings.

  • Bring appropriate documentation: Ensure you have the documents needed to:

    • Verify your identification; cannot be expired (All employees)

    • Verify your eligibility to work as listed on Form I-9, Section 2​ (All employees)

    • Verify your current vaccination record (Per job function)

    • List your current medications (Per job function)

    • Confirm your medical information (Per job function; refer to the forms emailed to you)

You will be assigned to the hub that is located in your geographic region but can choose whatever hub is most convenient for you when calling to schedule. ​​
